Date of admission: 31/08/21

HISTORY OF PRESENT ILLNESS- 
  • A 45 yr old female pt came with chief complaints of fever with chills since 3 days
  • Hopi - pt was apparently asymptomatic 3days back then she developed high grade fever associated with chills ,rigors
  • No c/o of cough, cold, 
  • No c/o of chest pain , palpitations, syncopal attacks
  • No c/o of sob , orthopnea ,pd
  • Nausea present 
  • no vomiting 
  • loose stools
  • No c/o of abdominal distention 
  • C/o of , burning micturition, decreased urine output 

PAST HISTORY
  • No h/o HTN,, CVA, CAD,TB, epilepsy.
  • K/c/o dm since 2 yrs on regular medication 

PERSONAL HISTROY
  • Decreased appetite
  • Bowel and bladder movements are regular
  • Non alcoholic , non smoker 
  • The patient is conscious coherent and cooperative
  • Moderately built and moderately nourished. 

VITALS
  • PR:80bpm
  • BP:110/80 mmHg
  • RR:16cpm
  • Spo2:99% at RA
  • GRBS: 560mg%

PHYSICAL EXAMINATION
  • Shape of abdomen: scaphoid
  • Abdominal tenderness is present in epigastric region 
  • Hernial orifices- normal
  • No free fluid
  • Liver and spleen - not palpable
  • Bowel sounds are present

CVS: 
s1 ,s2 heard 

RS:
 vesicular breath sounds are heard 

CNS: normal 

Post lunch blood sugar


Glycated hemoglobin 



X-RAY



HEMOGRAM




Provisional diagnosis:
Viral pyrexia with thrombocytopenia (?  Dengue)

59 year old female

This is a case of a 59 year old professor who is a resident of West Bengal. CHIEF COMPLAINT : Case of  Rheumatoid arthritis under medication.   HISTORY OF PRESENT ILLNESS- Patient was apparently asymptomatic an year and half ago.  Then she felt pain in her left wrist joint which was insidious in onset and gradually progressive in nature. It slowly progressed to the hands and shoulder on both the sides with an increased intensity causing difficulty in daily activities and sleep. She used painkillers which provided relief for sometime.  She underwent a physiotherapy for a month. She also took steroid injections twice in a local hospital in West Bengal but the pain reappeared later. 76 year old male with pedal edema

76 year old male farmer by occupation came with chief complaints of  Pedal edema since 20 days History of present illness  Patient was apparently normal  15 years ago he sustained an injury for right leg while working with tractor for which he developed ulcer and was hospitalized for 1 week while then he was diagnosed with Diabetes mellitus on medication.(Metformin 500 mg OD) 3 years ago he developed Blurring of vision in right eye undergone cataract surgery. 3 months ago he developed Blurring of vision in left eye. 25 days ago he developed abdominal distension. 20 days ago patient developed pedal edema in both limbs which was insidious in onset which is of pitting type extending upto the knee.
